Reproductive Health Care Information: Your patient portal may include reproductive (or potentially reproductive) health care information. Reproductive health care information is broadly defined as any health care that affects (or could potentially affect) the health of the individual in all matters relating to the reproductive system and its functions and processes.

Notice to Parents and Legal Guardians: Effective April 1, 2025, all parental/legal guardian, and proxy access will be automatically removed from the minor’s portal immediately or upon the minor’s 13th birthday. To reinstate access, a Proxy Request form will need to be completed by the minor and the parent/legal guardian. If you need medical records, please contact the HIS Dept at HIS-CustomerService@stillwater-medical.org or by calling 405.742.5737.

For routine testing and normal results, please allow your physician’s office 3 business days to contact you for follow-up. This gives your physician time to review and contact patients with critical/abnormal results first. Your patience and understanding are appreciated.

There’s an App for That! It’s called the Stillwater Medical “MHealth” App—the mobile app gateway is a free app available through your Apple Store or Google Play. “MHealth” replaces the need to bookmark the portal url on your smartphones and/or type in the web address.

How do I communicate my medical concerns?

Do not use the Stillwater Medical Center Patient Portal to communicate medical concerns for any reason. IF YOU ARE EXPERIENCING AN URGENT MEDICAL PROBLEM, PLEASE CALL 9-1-1 OR YOUR PHYSICIAN’S OFFICE IMMEDIATELY.

Some of my information is wrong. How do I correct this?

Any errors in personal information can be corrected by sending a change request through the portal. To correct errors in medical history information, please notify the Health Information Services department at (405) 742-5737 or his-customerservice@stillwater-medical.org.

How do I enroll?

First, your email address must already be a part of your SMC medical record and you will need your medical record number. If you don’t know your medical record number or you need to add your email to your record, call the Patient Portal Specialist at (405) 742-5752. You may also enroll in person by visiting Health Information Services Monday through Friday, 8am – 5pm.

How do I see my results?

Test results and radiology reports can be found by clicking the Health Record button on the portal home page. On the Health Record page, click either the “results” or “reports” buttons on the right. NOTE: Please allow 24 hours for results to be available.

Can I also see my family member’s health records?

Parents of children under the age of 18 may request and access a portal account for their children. Adults who act as another adult’s healthcare proxy may access the patient’s healthcare information once the patient has completed and submitted a Proxy Request form. Download the Proxy Request form here. If you have legal authority over an adult patient, you can request proxy access on the patient’s behalf. You will need legal documentation of your authority and must submit it in person at the Health Information Services Department at Stillwater Medical Center.

How can I update the email address in my medical record?

The email address in your record is the address that will be used for sending activity, resetting your password, and other communications. If you need to update it, please contact the Health Information Services department at (405) 742-5737 or his-customerservice@stillwater-medical.org.

How do I get the complete set of my medical records?

We have partnered with Sharecare to improve our flexibility for our patients and customers when requesting medical records. You do NOT need to come to the hospital or clinic, call, or email to get records. With this new relationship, you can request records online at your convenience using your computer or mobile phone.
