Medical Records

Requests for Health Information

Thank you for allowing Stillwater Medical to provide your patient care needs. We have partnered with Sharecare to improve our flexibility for our patients and customers when requesting medical records. You do NOT need to come to the hospital or clinic, call, or email to get records. With this new relationship, you can request records online at your convenience using your computer or mobile phone.



Patients and Legal Guardians

If you are a patient or legal guardian needing to request medical records, please click on the appropriate link below to complete your request for medical records. You will be required to provide a valid email address and upload a government-issued ID.

Hospital RecordsspacerClinic Records

NOTE: If you need BOTH Hospital and Clinic Records, please use the HOSPITAL RECORDS link above. We are still onboarding our hospital at Blackwell. If you need your records from this location, email us at HIS-CustomerService@stillwater-medical.org. If all required documentation is completed, request are usually available within 5-7 days of receipt of request.

Only the patient, parent/legal guardian, or the patient’s legal health care representative can sign the authorization form to release medical records. If you are requesting records on behalf of the patient or as the patient’s representative, please provide a copy of Guardianship or Durable Power of Attorney for Healthcare or Conservatorship.



Requests by Attorneys or Third Parties

If you are an attorney, insurance company, or any other entity requesting records from our facility, please click on the appropriate link below to upload your request along with the patient’s authorization.

What if I also need my Radiology Images?
Images are stored in a separate system and are not released by Sharecare currently. Please contact the Medical Imaging Department at 405-742-5446 for assistance. To expedite this process, you can download and complete the Authorization Form.
