Stillwater Community Action Team
Stillwater Community Action Team on preventing hospitalizations (SCAT) is a community centered team designed to innovate solutions to reduce and avoid hospitalizations in our community and surrounding areas. The healthcare team has developed community wide initiatives to help our patients understand how to manage chronic disease process, become knowledgeable about medications, and have their health information shared across the healthcare continuum from physician office to hospital to post-acute providers such as home health care, nursing homes and hospice agencies. The journey began with individuals from Stillwater Medical Center inviting other community healthcare providers into a proactive team to solve common concerns in our community.
