Our primary focus is helping you or your loved one maintain health and remain independent for as long as possible. This is accomplished through a variety of skilled nursing services, such as assessment, teaching, wound care, injections, IV therapy, mother/infant care, diabetic care, cardiac care and more. Skilled therapists may be utilitized to assist the patient with improvement in functional tasks of daily living and increased mobility. We provide services in the comfort of the patient’s own home, with a registered nurse available 24 hours a day.

Skilled Nursing
Providing assessment, teaching, wound care, catheter care, ostomy care, injections, IV therapy, mother and infant care, diabetic care, cardiac care and more.
Home Infusion Therapy
IV therapy, providing education and training, pain management, antibiotics, hydration and chemotherapy
Occupational Therapy
Instruction to help you increase your abilities to live independently through activities of daily living, such as dressing, feeding and self-care.
Physical Therapy
Rehabilitation services for stroke, cardiac and post-surgery patients, including treatments, exercises and instruction designed to improve the quality of your muscle tone, coordination, balance, strength, endurance and joint flexibility.
Speech Therapy
Evaluation, education and other services to those with speech and communication problems and/or swallowing difficulties.
Medical Social Services
Comprehensive counseling and coordination with other community agencies, public or private, among other services designed to round out your mental and emotional well-being.
Nutritional Counseling
Registered dietician instruction for new diets, including diabetic, cardiac and lifestyle accountability to improve nutrition and aid overall healing and/or health.
Personal Care
Home Health aides, supervised by professional employees, assisting with personal care: bathing, skin care, walking, basic exercises, and simple household tasks.
WISHH
WISHH is Women & Infant Skilled Home Health - a program specifically designed to provide post-delivery care in the comfort of your own home. This unique program allows a nurse come to you in the comfort of your own home. Your WISHH nurse can provide information on bathing, safety, lactation, height/weight check-ups, bilirubin levels, immunizations, tips for handling your baby and more. The WISHH nurse can even provide Synagis injections to help prevent respiratory syncytial virus (RSV).
WISHH has a maternal component to care for the mother in both the pregnant phase and the post-delivery phases as needs are determined. The WISHH nurse works in conjunction with the obstetrician and pediatrician to provide coordinated care in the home setting.
