Personal Health Records

A Personal Health Record (PHR) is a medical record created by you, the patient. You may have more than one physician, pharmacy, medication or health care provider. A Personal Health Record ensures you have one, complete record of all previous healthcare you have received and all care you currently are receiving. You collect, maintain and update a record of all medications and treatments from all health providers. This allows your doctors to make the most informed decisions to accurately diagnose and treat you.

A Personal Health Record can be paper-based, Internet-based or PC-based, whichever is most convenient for you and your family. Your personal health record takes a personal commitment but can prove to be an invaluable tool for you, your family and your care providers.
